Application Procedures for Practicum & Internship

Complete Practice Orientation

Starkville

• Send required e-mail to Clinical Coordinator
  Dr. Justice (cjustice@colled.msstate.edu) &
  Clinical Assistant Mikayla Herndon (mnh321@msstate.edu)
  (Name, MSU NET ID, Concentration, Place of practicum or internship you are applying for, semester and year you wish to enroll)
  • must be prior to official deadline

Meridian

• Send required e-mail to Clinical Coordinator
  Dr. Melissa Windham (mhw195@msstate.edu)
  (Name, MSU NET ID, Concentration, Place of practicum or internship you are applying for, semester and year you wish to enroll)
  •  must be prior to official deadline

Complete Application package

• Complete all information on Practicum or Internship application forms
• Proof of Liability Insurance
  • That covers the semester you are applying for (Practicum & Internship)
• Supervision Agreement form signed by all Required Individuals
• All required Signatures on the Application
  • On Site Supervisor
  • Academic Advisor
  • Coordinator Signature
• Site Supervisor Resume
• Give application package to Clinical Coordinator
  • must be prior to official deadline

Application Deadlines

• July 1st for Fall Fieldwork
• November 1st for Spring Fieldwork
• May 1st for Summer Fieldwork
• Applications must be submitted to the Clinical Coordinator for approval PRIOR to the official deadline
  • Application will not be accepted after the official deadline
